Client - Therapist Policies

All the information given on my INTAKE FORM is correct and up to date. I,client, take it upon myself to update any pertinent changes in my health during future visits. It is important for clients to inform their massage therapist of any allergies, particularly to oils or lotions, prior to the start of the session. Client should be provided without asking.

I understand that this massage is for stress reduction and relief of muscular tension or spasm, and is not a substitute for medical examinations or treatments.

I understand the therapist does not diagnose, treat,or prescribe for any illness, ailment, or disease. While the therapist may assist me in relief of physical or emotional symptoms, I understand that the therapist will not necessarily “fix” me.

I am aware that this is a non-sexual massage. Any misconduct or inappropriate behavior in that arena will result in the immediate termination of the massage with full payment due.

I understand that I am always in control of my body during my  session and will feel free to comment on      the comfort or discomfort of the amount of pressure or type of stroke used or body part should be avoid. I       can say “stop” at any time during the session.

It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently taken, and to provide written permission from the health care provider that the massage may be continued

The client must also keep the massage therapist informed of any change in health conditions and any recent injuries that need to be either avoided or excused from a physician that massage treatment may continue.
